Please include the following information and send it to our Admission and Career Support Center.

  1. Postal code & address.
  2. Name in kanji or alphabet & name in katakana in parentheses.
  3. Telephone number.
  4. Current high school name, prep school name (if applicable) or other institutions to which you are affiliated.
  5. Year or grade of high school (if applicable).
